Note: Application must be signed by you before being submitted.  You may sign the application using your own “electronic signature” which must be a copy of your actual signature; alternatively, you may print the application after completing it online, and then manually sign it.

Once completed, please either:
a)     scan and email, mail or fax the completed form, or
b)    if you are able to electronically sign the form, you may then simply click
    “Submit   Form” at the bottom of the application.

Applications that do not contain a true copy of your actual signature, whether signed manually or electronically, will not be accepted.

·         One driver and bike per policy.
·         Bikes over 30 years old will be insured for liability only.
     No physical damage coverage provided.
·         Bikes valued at over $40,000 can be insured for liability only.
     No physical damage coverage provided.
·         We are unable to provide coverage for bikes over 2500 cc's.

Complete the form in full and sign it.

Please ensure that you initial your acceptance or rejection of Uninsured Motorists and/or Underinsured Motorists coverage.

Provide a recent photograph of the motorcycle that represents the condition and style of motorcycle.

STEP 2:

Choose method of payment as shown on application form.
Add $60 policy fee to the calculated premium. Please note US Dollar currency only.
Credit Card users will be charged a credit card fee of 2.5%.

STEP 3:

Air Mail all documents, at least 45 days prior to your trip, to:

If you have less than 45 days prior to your trip, you may email or fax your application form to 866-768-7558 to begin the process. Please provide an email address or fax number to return a copy of your policy and proof of insurance card.
